Initial Evaluation

Before starting ovulation induction, a thorough evaluation is conducted to assess the woman's medical history, menstrual cycles, hormone levels, ovarian reserve, and any underlying conditions that may affect fertility. This evaluation may include:

  • Blood Tests: To check hormone levels such as FSH, LH, estradiol, and progesterone.
  • Imaging Studies: Transvaginal ultrasound to visualize the ovaries and assess the ovarian reserve.
  • Hysterosalpingogram (HSG): To evaluate the fallopian tubes and uterine cavity for any abnormalities or blockages.

 

Ovulation Induction (if needed)

In some cases, women may undergo ovulation induction with fertility medications to stimulate the ovaries and promote the development of multiple follicles and eggs. Medications used include:

  • Clomiphene Citrate (Clomid)
  • Letrozole (Femara)
  • Gonadotropins

Monitoring of Ovarian Response

Preparation of Sperm

On the day of the IUI procedure, the male partner provides a semen sample through masturbation. The semen sample is processed in the laboratory through a process called sperm washing, which:

  • Separates Motile and Healthy Sperm: From seminal fluid and debris.
  • Concentrates Sperm: To improve the chances of fertilization.
  • Removes Potential Contaminants: Ensuring only the best quality sperm are used.

 

Insemination Procedure

Once the sperm sample is prepared, the IUI procedure is performed as follows:

  • Positioning: The woman lies on an examination table, and a speculum is inserted into the vagina to visualize the cervix.
  • Insertion of Catheter: A thin, flexible catheter containing the washed sperm is inserted through the cervix and into the uterine cavity.
  • Sperm Deposition: The sperm are deposited directly into the uterus, bypassing the cervix and reducing the distance sperm need to travel to reach the fallopian tubes where fertilization occurs.

 

Post-procedure Monitoring

After the IUI procedure:

  • Resting Period: The woman may rest for a short period before resuming normal activities.
  • Possible Side Effects: Some women may experience mild cramping or spotting.
  • Luteal Phase Support: Progesterone supplements may be prescribed to support the luteal phase and improve the chances of embryo implantation.

 

Pregnancy Testing

Approximately two weeks after the IUI procedure, the woman undergoes a pregnancy test to determine if conception has occurred. If the pregnancy test is positive, further monitoring and prenatal care are initiated. If the pregnancy test is negative, the couple may discuss further fertility treatment options with their healthcare provider.
